Thomas Smith, 1st sergt., Co. K, 1st Mich. S.S., admitted to Harewood U.S.A. Gen'l Hospital, August 15, 1865, suffering from a severe and destructive inflammation, with loss of vision of both eyes, contracted on field; cause unknown. Constitutional state of patient otherwise good. This soldier has a congenital deformity of right forearm, and in this condition served faithfully for four years in the ranks, carrying his musket, and regularly performing his duties. Transferred. THOMAS SMITH.
