Manuscript, on paper, containing a collection of medical treatises and recipes. Includes: medical recipes and charms (ff. 1-57), gynaecological treatise (ff. 60-71), catalogue of plants and their medical properties (ff. 73-99), and an astrological treatise (ff. 103-114).

Layout: single column of around 30 lines.
Binding: bound in contemporary vellum. Severely damaged.

- Probably written and held at Maxstoke Priory (Warwickshire, England): front guard leaves from an account book of Maxstoke Priory and back guard leaves from draft of a quitclaim deed from Maxstoke Priory. Sixteenth-century signatures of Edward and Henry Cookes. Formerly owned by Mrs. H. Willes of Newbold Comyn, Leamington Spa. Included within archival box is a note from the British Museum describing the contents; it is dated 4 June 1957 and signed by P. D. A. Harvey, Assistant Keeper of Manuscripts. Purchased from Laurence Witten, booksellers. Presented to Yale Medical School Library in 1960 by Lucia P. Fulton.
