The plate design is such that it looks like the outside of a mausoleum, including columns, carvings, and other design. At the top are two portraits. Below is the dedication Bibliothecæ in Quibus tan quam Mausolæis Priscorum Sanctorum Reliquiæ Virtutis Plenæ Conditeæ Sunt. Additionally, enclosed is a letter dated 1922 that indicates Dr. Dane had stopped collecting bookplates.

To the left of the shield stands a skeleton with a caduceus it its feet. To the right, is an urn filled with flowers. This bookplate consists of a large mantelpiece over a fireplace with several books and objects featured throughout, including urns, candlesticks, matches, and a pipe. At the top is carved Christ's Coll[ege] Cambridge and St. George's Hospital. Just above the fireplace is the year 1899.
A picture of a caduceus in front of a shelf of medical textbooks. In blue ink beneath, over previously crossed-out text, is the name "K. A. Williams". The next line, written along the border below, reads "Box 278, Sterling Hall of Medicine, New Haven, Conn". The bookplate is probably a stock item in which the collector writes his or her name below.
