- At the center of the bookplate is a shield with an argent bordure. A quartered field with two designs—the first and fourth quarters with an argent field divided by a saltire and an argent chief with three cushions upon it. The second and third quarters feature an argent field divided by an argent bend—the top party features three heads of elk; the lower party features three crosslet fitchee. At the helm is a large knight's helmet backed by leafing; at the crest, atop a torse, is an griffin with the motto Vive ut Postea Vivas in a banner furled above. Beneath the shield is a small medal with Juncta in Uno Tria printed upon it.

At the crest is a carpenter's square with several instruments displayed upon it. Behind this is long, elaborate leafing surrounding the entire design. Beneath the shield, under a banner reading Ex Libris, is a kite with a key at center and a lightning bolt dividing the image. To the left of the shield is a large caduceus; to the right is an owl standing atop three books.

- The Yale insignia, including the phrase Lux et Veritas, at top center of a vertical rectangle surrounded by a decorative border. Beneath this reads "Gift of Dr. Lambert's nephews — Dr. Adrian Lambert B.A. 1930; Dr. John T. Lambert B.A. 1935; Dr. Samuel W. Lambert Jr. B.A. 1919; Dr. Dickinson W. Richards Jr. B.A. 1917." This bookplate is probably from the period when medical books were kept in a collection at Sterling Memorial Library, before the Yale Medical Library was built. Alexander Lambert received his BA from Yale in 1884, his Ph.B. from Yale in 1885, and his M.D. from Columbia in 1888.
