- Harewood U.S.A. General Hospital,, Hospital Number, 20,474., R. B. BONTECOU,, Surgeon U.S. Vols., In charge., William Brown, Corp'l, Co. I, 1st Mass. H.A., aged 34, was admitted to Harewood U.S.A. Gen'l Hospital, April 5th, 1865, suffering from gun shot wound of posterior part of lower third of left forearm, injuring ulna. Wounded, March 31st, 1865, at the battle of Petersburgh, Va. Amputation of left forearm, upper third, by a circular incision. Operation performed by Surg. R. B. Bontecou, U.S.V., April 15th, 1865. Condition of injured parts at time of operation very bad, gangrenous and sloughing. Constitutional state of patient very poor. Patient did tolerably well after the operation, till about April 22nd, but from that date he suffered severely from diarrhea, and gradually sank, notwithstanding the free use of astringents, opiates and a general supporting treatment throughout., and William Brown.

- Harewood U.S.A. General Hospital,, Hospital Number 20,870., R. B. BONTECOU,, Surgeon U.S. Vols., In charge., Walter Wheeler, Private Co. B. 91st N.Y. Vols., age 37, was admitted into Harewood U.S.A. General Hospital, April 19th 1865, with gunshot wound of the right side of the head. The wound was received before Petersburg, Va., April 2nd 1865. The ball entering above the anterior zygomatic articulation and passing through temporal fossa emerged just above the external meatus. April 22nd 1865 he had a slight hemorrhage from the common temporal artery which was ligated. An incision was made between the wound of entrance and exit, when it was found that a small portion of the bone at the point of exit was denuded of pericranium. A portion of bone was subsequently removed and the patient is now (June 17) fully recovered., and Walter Wheeler.

On admission to this Hospital, the patient had so far recovered as to be able to sit up and move about—the bone having united firmly; wound still open and discharging in consequence of necrosed bone; his prospects for a useful limb are good. and TIMOTHY F. - Harewood U.S.A. General Hospital,, Hospital Number 20.611., R. B. BONTECOU,, Surgeon U.S. Vols:, In Charge., Thomas W. Hollen, private Co. H. 205 Pa. Vols., age 28, admitted April 5th 1865, to Harewood U.S.A. General Hospital from City Point, Va., with gun shot wound through right wrist joint, also upper third right arm, flesh. Wounded at Petersburg, April 2, 1865. When admitted wrist and hand were very much swollen and inflamed, but by the application of cold to the parts all inflammation had ceased. April 20th, the wrist and hand became again swollen and very much inflamed, and all remedies failed to subdue it, sinuses having formed on the dorsal surface of hand and wrist. The sinuses were relieved of pus, and fore arm enveloped in bran; sinuses continued to form, having involved the cellular tissue of elbow joint. April 28th, by bandaging from shoulder to elbow joint, the inflammation was subdued. Very little discharge from wound at present time (May 1st.) Gangrene attacked the hand, but soon disappeared., and Thomas W. Hollen.

- Thomas Smith, 1st sergt., Co. K, 1st Mich. S.S., admitted to Harewood U.S.A. Gen'l Hospital, August 15, 1865, suffering from a severe and destructive inflammation, with loss of vision of both eyes, contracted on field; cause unknown. Constitutional state of patient otherwise good. This soldier has a congenital deformity of right forearm, and in this condition served faithfully for four years in the ranks, carrying his musket, and regularly performing his duties. Transferred. and THOMAS SMITH.

- Thomas Miller, private, Co. G, 116th Pa. Vols, aged 18, admitted to Harewood U.S. Gen'l Hospital, August 15, 1864, convalescent from gunshot wound, left thigh, ball entering and fracturing femur at junction of middle and upper of thigh, past through exit inner side, near the tuberosity of ischium. Wounded, May 5, 1864, at the battle of Wilderness, Va. On admission to this hospital, the patient had so far recovered as to be able to be about on crutches. The patient, however, states, that at time of injury his constitutional state was very good, although wound very painful. Severe hemorrhage followed the injury [and] great tumefaction of the whole limb. The treatment consisted of placing the limb in a fracture-box counter extension, and supporting throughout. Limb shortened about [?] inches and three-fourths; patient otherwise in good condition and in a fair way of having a useful limb. and THOMAS MILLER.

- Harewood U.S.A. General Hospital., Hospital Number 20,295, R. B. BONTECOU,, Stephen D. Myers, Sergeant Company E. 15th N.Y. Cav., was admitted to Harewood U.S.A. General Hospital April 5th 1865, from City Point Va., having a gunshot wound of the scalp, with fracture of the outer table of the calvarium. The precise locality of the injury is in the course and to the right of the sagital suture, between the two parietal bones, one and one half inches from the posterior fontanelle. The patient complained of constant headache, dizziness and a tendency to drowsiness. He was put on Low Diet, the use of all stimulants interdicted, quietness and the recumbent posture recommended. Under this treatment the patient gradually rallied from his lethargic condition, the suppuration diminished the flesh is granulating, and the prospect of speedy recovery is very fair. Shady Grove, Va., June 2nd, 1864. Cold Harbor, Va. June 3rd, 1864. Front of Petersburg, Va., June 17th, 1864. Promoted from Corporal to Sergeant Nov. 1st, 1864, for gallantry in action July 30th, 1864., Serg't. Samuel C. Wright, Co. E, 29th Mass. Vols., aged 21, was admitted to Harewood U.S.A., Gen'l Hospital, August 1st, 1864, with gunshot wound, received June 17th, 1864, at battle of Petersburg, Va. The ball, a Minnie, struck the right orbit, fracturing the bone. The case progressed favorably, the patient making a good recovery. Discharged Feb. 13th, 1865., Serg't. Samuel C. General Hospital, April 19th, 1865, with gunshot wound of the abdomen. The ball entered over the left hypochondriac region, passed transversely toward the spine, and emerged about 6 inches from point of entrance. There have been no symptoms indicating injury of internal organs. The ball was of the conoidal variety. The wound was received at Sailor's Creek, Va., April 6th 1865. The patient did well and was transferred to Philadelphia, Pa., May 18th, 1865., Samuel C. Pew., and Surgeon U.S. Vols., In Charge.

- Dec. 10th. Abscess midway between nipples and clavicle was to-day opened, and considerable pus escaped; a probe could be passed under the clavicle and all through the Axilla, which is one abscess, I cannot discover the ball, however. The patient walks about and is on his feet the most of every day., Dec. 1st, 1862. Opened the abscess above the clavicle and a considerable quantity of pus escaped. I was unable to discover the ball as I hoped. Poultice applied to this abscess., Dec. 29th. Since last report this man has gained flesh, his wounds are nearly closed and with but little discharge. Sent north to-day per Steamer "Star of the South.", Nov. 10th. The abscess in the neck and underneath pectoral muscles, having discharged through the shoulder wound, the poultice was discontinued, and the surfaces of the deltoid, which had torn from the sutures and gaping were approximated with straps, and the whole enveloped in lint and dry tow., Nov. 18th. Doing well; discharge less and the abscess of the neck and breast showing less indication of suppuration., Nov. 25th. Suppuration quite free, and indistinct fluctuation above the clavicle, and also in the pectoral region, midway between nipples and clavicle, left side. Some wads of clothing were removed by the dresser to-day; they were projecting from the gunshot wound. The wound is drawn together gently by straps, and charpie applied. The patient is allowed his clothes and to walk about, which he does, and feeling better than when in bed., Nov. 6th. Some increase of pain, and evidently increased swelling in the infra and supra-scapular regions, with tenderness and indistinct fluctuation, but no discoloration. The clavicle is difficult to recognize, the parts are so tense and swollen. Ordered poultice to cover the whole shoulder and neck, and a laxative of ol. ricini. Half diet., Nov. 7th. Doing well; discharge considerable; and ordered full diet, with one-half pint of beer at noon and night., Nov. 8th, Doing well, but not having slept well last night, was ordered pulv. dover, grs. x., and Private Robert Wilson, Co. D, 6th Conn. Vols., aged 24 years, wounded at battle of Pocotaligo, Oct. 22nd, and admitted to Hospital No. 1, Oct. 24th, 1862, with gunshot wound of left shoulder, the ball entering anteriorly, mid way between acromion and coracoid processes, and grazing the internal aspect of the head of humerus deeply at its junction with the neck of that bone. No other wound was made and the ball was not discernible through the wound. A considerable swelling under the pectorals, near the clavicle, with tenderness and green discoloration, rendered it possible that the ball was lodged there. Motion of the arm was painful; no crepitus, however. The swelling extended to the shoulder and arm, and some anasarca of the forearm; radial pulse good. Wet dressings were kept applied until the 28th inst., when ice bag and cerate dressings were substituted, and, Nov. 11th, swelling having considerably subsided, I resected the head of the humerus by a transverse incision of the deltoid, and a perpendicular one from it down the posterior part of the arm. The chain saw was applied after dissecting back the soft parts to a very small extent, and without dividing the attachment of any other muscle, the long head of biceps having been shot away; ice and cerate was continued, with oiled lint in the wound. When the joint was laid open, pus in considerable quantity came away. A track was discovered leading inwards under the clavicle, but the ball could not be found. The swelling has very much diminished, and he expresses himself very well. The case is doing well at this date, Nov. 5th.

Vols, was admitted to Harewood U.S. Gen'l Hospital, April 16th, 1865, having two sabre cuts across the top of the head, received near South-side Railroad, Va., April 10th, 1865. The anterior wound was near, and in line with the coronal suture. The posterior one, about one inch and a half from the first and parallel with it. The bone was not denuded. The wounds healed rapidly. May 29th, the patient, for the first time complained of severe headache, and intolerance of light, and the anterior wound soon after re-opened discharging unhealthy pus. - Gunshot wound. Disarticulation at shoulder., Martin Burke, sergeant, Co. K, 15th N.Y.H.A., aged 29, admitted to Harewood U.S.A. Gen'l Hospital, September 13, 1865, suffering from amputation of left arm, upper third. Wounded before Petersburg, June 25, 1864; operation performed at Lincoln Hospital—particulars unknown. The patient states that at time of operation his constitutional state was good; but that parts became soon afterwards erysipelatous, extending through the whole chest, gangrenous and considerably sloughing, a destruction of soft parts, and the shaft was entirely removed by disarticulation at shoulder joint, by straight incision, March 29, 1865. The bone became necrosed nine months after the amputation; several abscesses had since collected in stump, and up to date, although the patient is good health, there are still several fistulous openings, with considerable discharge of pus. Treatment disinfectant, astringent lotions, and supporting throughout with liberal use of iron internally., and MARTIN BURKE.

The ball entered about the middle of the palmar surface, and emerged at the dorsal, fracturing the third metacarpal bone. The wound was received near Petersburg, Va., March 15th 1865, and was caused by the conoidal ball. The use of the middle finger is much impaired, in consequence of the necessary removal of loose spicula of bone, and gangrenous sloughing of tendon of flexor muscle. Hospital Oct. 7th, 1864, with amputated left thigh, lower third. The wound was received Sept. 3Oth, 1864, near Petersburg, Va.; nature of missile not positively known, but supposed to have been the conoidal ball. The patient states that the ball passed through the knee-joint. On the 29th of March, 1865, a sequestrum measuring about six inches, was drawn from the end of the stump. - Harewood U.S.A. General Hospital., Hospital Number 15,877., John Harvey, Private, Co. F, 29th Mass. Vols., aged 19, was admitted to Harewood U.S.A. Gen'l Hospital, June 21st, 1864, suffering from gun shot wound of left forearm, the ball entering at upper third and passing upwards, grooving the head of humerus and fracturing left cavicle. Wounded, June 17th, 1864, at the battle of Petersburgh, Va. Resection of head of humerus, and of about three inches of left clavicle which was fractured, denuded and protruding. The ball was extracted from under fractured cavicle on the 28th of June. Operation performed by Surg. R. B. Bontecou, U.S. Vols., July 17th, 1864. The condition of parts at time of operation was tolerably good, but patient was weak from excessive suppuration. Gen'l Hospital, April 5th, 1865, suffering from gunshot wound of back, dorsal region, ball entered the chest opposite the fourth dorsal vertebræ, between the spine and scapula. Wounded, March 25th, 1865, at the battle of Petersburgh, Va. On admission, the condition of injured parts and constitutional state of patient were good; did very well for a while, wound healing kindly, but towards the early part of the month of May the chest became enormously distended with effusion, harassing cough, anxiety of countenance, oppressed breathing and symptoms of empyema. An operation being necessary to relieve the patient paracentesis thoracis was performed by Surg. R. B. Bontecou, U.S. Vols., May 9th, 1865, by freely opening the chest at right posterior and lateral aspect, between eighth and ninth ribs, and about six pints of sanious pus removed, no anæsthetic used; the patient felt at once relieved, did remarkably well after the operation; was doing well and in tolerably good condition when he deserted from this hospital, May 28th, 1865. - Henry Krowlow, Private, Co. A, 66th N.Y. Vols., aged 18, was admitted to Harewood U.S.A. Gen'l Hospital, April 5th, 1865, suffering from gun shot wound of right leg, the extent of injury unknown. Wounded, March 31st, 1865, at the battle of Petersburgh, Va. Amputation at right knee joint, antero-posterior flaps, on the field, April 1st, 1865, operator unknown. The condition of injured parts and constitutional state of patient at time of operation unknown, but on admission to this hospital they were tolerably good. Patient did very well for awhile, the stump improving, but suffering from erysipelas of the face. The stump soon afterwards became gangrenous, the disease destroying the soft parts. - George W. Northard, Sergt. Co. G., 2d Ohio Cav., age 23, wounded, April 1, 1865, at the battle of Petersburg. Admitted to Harewood U.S. General Hospital, April 1, 1865, with gunshot wound of right knee. The ball entered just above the patella through which it passed, fracturing it into six fragments—passed downward and outward, fracturing external condyle of femur, and escaped. On the 8th of April he was etherized, and Surg. R. B. Bontecou, U.S.V., removed the fractured patella and a fragment of the external condyle. The constitutional state of the patient at the time of the operation was very good. After the operation the limb was placed in a box containing bran, and the patient put on low diet. Subsequently the limb was placed upon a light open splint, extending from the tuberosity of the ischium to a point near the heel, and suspended by a cord; and the patient put a nutritious diet with wine and porter. On the 13th of April he was transferred to a tent ward. Up to this time he suffered considerable pain in the joint, and was very restless and irritable. Gen'l Hospital, September 12, 1865, suffering from great deformity of right limb the result of gunshot wound, right thigh, upper third, through trochanter major, severely fracturing femur. Wounded at the battle of Southside Railroad, Va., April 1, 1865. On admission to this Hospital, the patient had so far recovered as to be able to sit up, and constitutional state was tolerably good. The fractured parts had firmly united, but with great deformity; wound of entrance also healed; but there are still some discharges from sinures in thigh, and small fragments of necrosed bones are daily removed. - Fritz Siebe, private, Co. D, 139th N.Y. Vols., aged 23, admitted to Harewood U.S.A. Gen'l Hospital, June 18, 1864, suffering from gunshot wound of abdomen, ball entering right anterior side, below the 10th rib, emerging behind, and about 1 1/2 inches from spine. Wounded June 3, 1864, at the battle of Cold Harbor, Va. On admission, the patient was in a very feeble condition; the discharge from the wound consisted of feces, mixed with greenish streaks from the ascending colon and liver; complained of pain in abdomen, increased by pressure; discharge per rectum, scanty. Treatment, cold water dressing over the abdomen, opium internally, and light, nourishing diet. July 1—pain in abdomen diminished, but discharge the same; July 15—patient complains of occasional colic pain; August 1—patient improving; discharge the same up to August 15, but from that date the patient began to improve. September 1—the wound of exit healed; discharge consisting of a thin yellowish serum. September 14—wound entirely healed; colic pain, now and then recurring, but able to be about. - Fredrick Hohmann, Private, Company H, 1st Michigan Cavalry, age 27, wounded at Petersburg April 2nd 1865, was admitted to Harewood U.S.A. General Hospital, from City Point Va., April 5th 1865, with gunshot wound of thigh immediately above the knee joint, the ball entering on the upper surface of internal condyle of right knee, passing half an inch above the knee and parallel with it, to external condyle. When admitted, the patient was in good condition, with the exception of the parts in vicinity of the wound being very much swollen and inflamed, the joint not being implicated, the limb was kept perfectly dry and quiet. - Dennis Sullivan, Private Co. E, 2d Va. Cav., aged 21, was admitted to the Harewood U.S.A. Gen'l Hospital, April 19, 1865, with gunshot wound of the scalp, anterior aspect, over coronal suture, denuding bone of its pericranium. Previous to his admission he was attacked with chills, which continued to occur at intervals of about twelve hours, varying somewhat till April 25th. The sulph. of quinine was freely administered in full doses but without any appreciable effect. During pyrexia, pulse ranged between 90 and 100. April 24 the lower lobe, right lung, was discovered to be slightly congested April 26th the lung was now involved, with great pain in the cardiac region. Upon auscultation the bellows sound was distinctly heard, and occasionally the regurgitant murmur, the pulse rising rapidly from 90 to 156 per minute. Ten o'clock P.M., of same day, patient became comatose. Shortly after he was trephined by Dr. R. B. Bontecou, Surg. in charge of Hospital, when the external table of the cranium was passed, pus was found to exude from diploic structure, upon the latter being perforated, the singular circumstance presented itself, that while the outer table was uninjured the internal was fractured. The opening being enlarged the portion of fractured bone was removed with the forceps, being nine lines in length, and six in breadth. During the operation the patient seemed almost entirely unconscious, in which state he remained until he died, on the next morning, April 27th, 1865. The operator discovered, as has already been anticipated, and as he had correctly diagnosed, that the patient was suffering from abscess of the brain., Dennis Sullivan., Harewood U.S.A.
