"A doctor (left), grotesquely obese, stands in profile regarding with a satisfied smile his dead or dying patient, who leans back in an arm-chair with closed eyes. The patient, old and emaciated, wears night-cap and dressing-gown over breeches and stockings. Behind him are the curtains of his bed (right). At his side is a round table with a bowl, medicine-bottle, and a paper: 'Prescriptions, Bolus, Blisters'. On the ledge of a casement window is a close array of medicine-bottles. The doctor says: "My Dear Sir you look this Morning the Picture of health I have no doubt at my next visit I shall find you intirely cured of all your earthly infirmitys." "Grenville (left) stands in St. James's Park in profile to the right, firing a gun with a wide barrel at a group of winged money-bags, &c, flying in the upper right corner of the design, rising on smoke issuing from the chimneys of 'Buckingham House'. A pen in his round hat indicates his Secretaryship. Dead game hangs from his person: a hare whose body is a money-bag inscribed 'Secretaryship of State £7000 pr Ann.', a goose, whose similar body is inscribed 'Sinecures £9000pr Ann.', and two goslings, respectively 'Private Pension £4000' and '£3000 pr Ann.' The central bird at which he fires at close range is a winged document: 'Ranger of the Park'; winged bags are '75000 pr Ann', '3000 pr Ann', and '7000 pr Ann'. All these he hits. A winged ducal coronet is flying up towards the line of fire. A group of dogs with human faces surrounds him; they lick his boots, look up at him expectantly, or watch the shot; one has a collar inscribed 'Whitehall'. The water expands and falls by billowing stages into a sea, the horizon (l.) inscribed 'Unfathomable Sea of Taxation'. In this sea 'cormorants', with human heads and huge beaks (projecting from their profiles) and pelican-like pouches, are greedily fishing, while a rowing boat founders, throwing into the water John Bull, who drops an oar inscribed 'William Pitt'; only his head (submerged up to the mouth) and arms emerge. On the water's edge in the middle distance stands Buckingham, wearing a ribbon and swallowing three large fish. Little Lord Derby beside him stoops for one small fish. In the sky a flight of countless birds makes for the fish.
